Pathways to Success

Pathways to Success is a program for Medicaid enrolled children under the age of 21 in Illinois who have complex behavioral health needs and require intensive home and community-based services. The Pathways to Success team provides training and coaching to the designated Care Coordination and Support Organizations provide these services throughout Illinois through the following teams:

System of Care (SOC) Team

The System of Care team is responsible for providing training, coaching, and on-going support for Illinois agencies (Care Coordination and Support Organizations) involved in the Pathways to Success Program.  This team is responsible for aiding in the implementation of High Fidelity Wraparound and Intensive Care Coordination.  Trainings provided include but are not limited to:

  • Care Coordination              
  • Supervision and Coaching
  • Individualized Plan of Care Development
  • Integrating Multiple Perspectives
  • Developing Family and Natural Supports
  • Facilitating Child and Family Team Meetings, and
  • Consensus Building Within Teams

The SOC team provides monthly, collective coaching calls to providers and works to create individualized, organizational coaching plans with each provider to collaboratively support implementation of wraparound and care coordination across the state of Illinois.

Family Advocate Team

The Family Advocate Team consists of team members with lived experience as the primary caregiver of a child or adolescent with emotional/behavioral health challenges who have had to navigate multiple child serving systems.  The Family Advocate team provides leadership, on-going coaching, and support for Community Mental Health staff, provide oversight of fidelity to the family peer support model and serve as Subject Matter Expert for family peer support role. The Family Advocate Team also provides internal support through curriculum development and perspective to ensure that curriculum is develop and implemented with Family Voice and Perspective.

Intensive Home Based/Therapeutic Mentoring Team

The Intensive Home based/therapeutic mentoring team is responsible for providing training and ongoing support for Illinois agencies involved in the Pathways to Success Program. Trainings provided include introductions to:

  • Intensive Home-Based Services
  • Therapeutic Mentoring
  • Functional Behavioral Analysis, and
  • Managing and Adapting Practice (MAP)
    • 5 Day MAP Credentialing Course
    • MAP Supported User Training
