GENERAL REQUIREMENTS:

  1. Conduct face-to-face assessments of alleged victims of abuse, neglect, or exploitation within specified time frame.
  2. Conduct comprehensive assessments of alleged victims of abuse, neglect, or exploitation.
  3. Provide casework and follow-up services to ensure long-term support and intervention to prevent further abuse, neglect, and/or exploitation of the victim.
  4.  Other duties as assigned within the guidelines of this position.

ESSENTIAL FUNCTIONS:

  1. Receive and document reports of abuse, neglect, or exploitation of older adults and persons with disabilities.
  2. Conduct a case conference with the identified protective services supervisor at the point of intake, after initial face-to-face contact, when making substantiation decision, when developing the care plan, at case closure and at any other time deemed necessary by supervisor.
  3. Provide follow-up services as appropriate to client needs at a minimum of two face-to-face contacts and one phone contact every three months for a period of up to twelve months following intervention.
  4. Document all activities on required forms adhering to time frames and policies of the APS program.
  5. Make referrals for appropriate follow-up services, such as case management, ombudsman or other community based services.
  6. Ability to relate well to older individuals and persons with disabilities respecting their privacy, dignity and right to self-determination.
  7. Must display strong and independent problem-solving skills.
  8. Ability to prioritize and respond to client needs in accordance to specified time frames.
  9. Ability to work effectively with co-workers and other community agencies.
  10. Attend staff meetings, in-services, conferences and workshops as approved or recommended by supervisor.
  11. Be an active member of the APS Multi-disciplinary Team.
REQUIREMENTS:
  1. Bachelor of Science in Nursing Degree (BSN); or a Bachelor’s degree in health, social sciences, social work, health care administration, gerontology or criminal justice is required for this position. All degrees must be from a regionally accredited educational institution which has U.S. Department of Education approval.
  2. One (1) year of experience in health or human service is required.
  3. Ability to relate well to persons with disabilities and older individuals respecting their privacy, dignity, and right to self-determination.
  4. Demonstrated ability to work with others and take direction in a dynamic, fast-paced, and diverse environment.
  5. Demonstrated ability to prioritize a number of requests for services assuring that all requests are responded to in a timely manner.
  6. Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams.
  7. Strong verbal and written communications skills.
  8. Passage of complete background clearance and Tuberculosis (TB) Test.
  9. Catholic Charities, Diocese of Joliet, has adopted and follows the Codes of Ethics established by Catholic Charities USA, the National Association of Social Workers and the American Psychological Association.   10. A valid driver’s license, reliable means of transportation and proof of liability insurance is required.
