Job Description
TYPE: FULL-TIME (37.5 HOURS)
LOCATION: COMMUNITY-BASEDSUMMARY
Crisis Specialists perform crisis intervention, crisis assessments and case management services for any individual who is experiencing a mental health crisis as part of the Crisis Care System. The position will assist designated co-response agency/department with mental health related calls to link individuals and families to necessary services in efforts to link clients to the most appropriate services in the least restrictive environment. This position will provide crisis co-response, coordination, mentorship/education, service advocacy in crisis incidents. This position will provide some coverage during weekdays, weekends, overnight, and holiday shifts.

Clinical Responsibilities

  • Responsible for accepting crisis calls from the 988 Crisis Cares System line and from assigned co-responding agency/department therein providing a crisis screening to client at location requested.
  • Responsible for providing mobile Crisis Response, Crisis Intervention, Case Management, and other Rule 140 services.
  • Responsible for completing accurate and clinically sound assessments to assess for risk, presenting problem, medical history, social history, self-care habits, family system and other related areas to be able to form a recommended disposition with level of care and safety planning.
  • Responsible for identifying and assisting in overcoming barriers to on-going care that will assist the individual with managing symptoms.
  • Must complete any necessary agency documentation to include intake forms and releases of information for individuals seen through the Crisis Care System.
  • Responsible for coordinating with assigned agency/department regarding individuals needing follow up services.
  • Responsible for networking with collaterals on behalf of crisis/active clients, including: referral and linkage, facilitation of level of care determined, and the advocacy of a client’s rights within the education, legal, state and mental health systems when indicated.
  • Responsible for completion of and meeting deadlines for all clinical paperwork for active clients and crisis clients in compliance with Rule 132 and Rule 140.
  • Fulfill agency requirement of training and documenting all training attended and must attend all mandatory agency in-service trainings
  • Responsible for participating in Utilization Review meetings.
Supervision
  • Responsible for discussing cases with supervisor in clinical supervision.
  • Responsible for reporting to supervisor on an on-going basis potentially problematic situations/cases, and emergency or crisis situations.
  • Must attend monthly group supervision and responsible for discussing cases with supervisor in clinical, individual and/or group supervision as need arises
Administrative
  • Responsible for completion of and meeting deadlines for all clinical paperwork for cases, including: Direct Service Notes, Case Closings, IM+CAT, involuntary petitions, Universal Screening Assessment, safety planning, and other clinical paperwork necessary or appropriate for maintaining case files according to agency policies and procedures.
  • Responsible for completion of and meeting deadlines for all tracking reports and performance measures.
  • Responsible for completion of and meeting deadlines for all agency administrative paperwork including time sheets, expense forms, insurance documentation, etc.
  • Responsible for attending staff meetings, clinical staff meetings and program meetings.
  • Responsible for serving on agency and/or program committees when requested by supervisor.
  • Maintain FSA equipment.
Community Education/Public Relations
  • Responsible for educating the community regarding the Cares Crisis System and 988 in the service area.
  • Responsible for facilitating positive networking relationships with community service providers and/or networks as assigned by supervisor.
  • Responsible for working as a team with assigned location and following procedures and protocols of host environment.
Other Duties/Special Assignments/Projects
  • Be available to help develop materials and participate in special community education projects/presentations as requested by administrative staff.
  • Pursue and maintain licensure and/or certification that are available and appropriate to educational/professional background.
  • Perform other duties as assigned by supervisor.
  • Perform incidental business-related travel as needed.
  • Participate in agency Risk Management and Continuous Quality Improvement activities.
  • Perform job responsibilities with a positive attitude

  • Participate in projects or other duties as assigned by supervisor.

  • The incumbent will be sitting in car for travel for several minutes every day. The incumbent is frequently expected to walk, sit, stand, bend and drive. The incumbent may also need to climb stairs on occasion. Must be able to lift up to 35 lbs. on occasion.
  • The work environment is mostly field based and will have varying temperatures and noise level. The incumbent can expect to work in client’s homes, schools, public libraries, community agencies, and some in office time.
  • The physical demands and work environment described here are representative of those that should be met by an employee to perform the essential functions of this job successfully.
TRAVEL
This position requires extensive travel in Kane and Kendall County and occasionally to areas outside of that.BENEFITS

REQUIREMENTS
Education And Experience
Must have Bachelor’s Degree in Social Work, Counseling, Psychology or similar field, Master’s preferred. Must be flexible, independent and able to multi-task effectively. Preferred experience in working with individuals with mental health issues and mental health crisis intervention. Preferred to be bilingual in Spanish speaking but not required.
