Provides social work services to patients and families within assigned areas in order to meet identified psychosocial needs, as well as to enhance problem-solving and coping capacities and to facilitate timely discharge by arranging post-discharge services. The social worker provides psychosocial assessments, supportive counseling, crisis intervention, and financial planning and works collaboratively with all other members of the healthcare team, ensuring clear communication and care planning to all patients.

CERTIFICATION & LICENSURE REQUIREMENTS
Licensed Clinical Social Worker (LCSW) Illinois or Licensed Social Worker (LSW) Illinois. Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.
Ability to coordinate multiple projects concurrently and establish priorities. Possesses strong planning and organizational skills. Self-motivated. Requires flexibility in hours and job duties. Ability to work effectively with diverse individuals and groups.
