Job description
Position Overview:
The Cunningham Township Supervisor's Office (CTSO) employs an experienced Bridge to Home
Housing Case Manager who provides trauma-informed care for residents in our Bridge to Home
housing program. Residents include women, survivors of sexual violence, LGBTQ+ identified
residents, seniors, people with disabilities, and other vulnerable residents. This position
supports individuals to stabilize from the crisis of homelessness by addressing health and
wellness issues, increasing income, connecting to community support, and securing appropriate
housing.
Essential Duties:
● Screen individuals referred to the Bridge to Home Program to assess eligibility.
● Intake and assess participant needs, creating individualized service plans for care
and exit strategies.
● Conduct weekly meetings with participants, assessing progress, setting goals, and making
interventions.
● Support participants with income, budgeting, credit repair, and rental history as needed.
● Attend medical appointments, housing-related appointments, and court hearings with
participants as needed.
● Coordinate referrals to other community resources to meet additional needs.
● Support successful exits with housing navigation, reunification, or support accessing
inpatient services.
● Build partnerships with outside agencies providing referrals and support.
● Build rapport with individuals by utilizing a strength-based, trauma-informed lens.
Other:
● Maintain complete, accurate, and confidential participant files.
● Maintain up-to-date and thorough documentation.
● Track and report on program outcomes.
● Make recommendations for program improvements.
● Participate in weekly team meetings.
● Participate in staff training and professional development opportunities.
● Other duties as assigned.
Qualifications Include:
● LSW or MSW, or Bachelor’s degree in a human services field plus 3 years of experience,
or commensurate work experience.
● Ability to support individuals facing homelessness, hunger, substance abuse, or
mental and physical health issues, using a trauma-informed approach to care.
● Deliver services through a trauma-informed framework that emphasizes client
strengths and resilience.
● Ability to effectively organize, complete, and evaluate multiple
assignments.
● Excellent oral and written communication skills.
● Strong work ethic, good judgment, and a results-oriented approach.
● Ability to follow policy and procedures, and hold participant information in strict
confidence.
● Ability to adapt to changing environments and new technologies.
● Digital office skills including: Windows, Word, Excel, Outlook, Google Docs.
● Demonstrated ability to work effectively and respectfully with people of all economic,
racial, and gender identities.
● Must have a valid driver's license
● Spanish or French language proficiency is not required, but a plus.
