Independent Living Specialist

Job Overview:

Persons Assuming Control of their Environment, Inc. (PACE, Inc.) offers services which assist persons with disabilities in achieving and/or maintaining independence. Our goal is full participation in the rights and responsibilities of society for persons with disabilities. PACE, Inc. is looking for full time Independent Living Specialists (ILS) and is hiring multiple positions; ILS-Personal Assistant/Personal Support Worker, ILS-Low Vision, and ILS-Housing. Must be able to flex hours to different times of day as needed. Must be able to lift program promotion materials, equipment and transport to five-county area support groups (Champaign, Douglas, Edgar, Piatt and Vermilion). Must have reliable vehicle, occasional weekend/evening hours.

Job Duties:

• Supervise volunteers as needed.
• Assist in providing consumers facilitation.*
• Assist in identifying specific resource needs of consumer.*
• Operate and update databases.*
• Assist consumer to explore other services to increase and/maintain independence.
• Provide consumers orientation: Independent Living Philosophy, PACE & Home Services.*
• Assist consumers in identifying Independent Living needs through Independent Living assessment.*
• Assist consumer design Independent Living plan, contact consumer & document progress every 3 months.*
• Assist consumers in developing self and systemic advocacy skills.*
• Provide advocacy support as necessary. *
• Assist in coordinating peer counseling opportunities.*
• Assist in recruitment and program promotion.
• Provide Information and Referrals.*
• Achieve contract goals, maintain records and reports.*
• Coordinate satisfaction surveys and make outcome calls timely.*
• Assist in achieving grant goals pertinent to the positions.*
• Other duties as assigned.

Work Environment:

• Monday thru Friday 8am-5pm with an hour for lunch. Must be able to flex hours to different times of day as needed. Occasional evening and weekend work required.
• Travel for program outreach and events are required.
